Insurance/Fees

We are a fee for service practice and do not work with any insurance companies. We can only accept patients with commercial insurance or Medicare patients that have a secondary insurance as prescription drug coverage can only be through one of these. Currently, our office cannot see any patients with Medicaid.

Letters

Any request for Dr. Blanchfield to write a letter of any subject for any purpose will incur a fee for completion. Please fill out a Letter Request form and allow four weeks for processing. If your letter is due before the four week time frame, we will attempt to accommodate your due date, but cannot offer any guarantees as letters are frequently requested from Dr. Blanchfield.

Forms

Any request for Dr. Blanchfield to complete a form for any purpose must be brought / sent to the office for completion, is subject to a four week processing period once received, and will incur a fee for completion. If your form requires you to fill out any portion, this must be done before the form is brought to the office for completion. If your form is due before the four-week processing period, we will attempt to accommodate your due date, but cannot offer any guarantees as forms are frequently requested from Dr. Blanchfield.

Prior Authorizations

If required by your insurance company, we will complete a prior authorization request. Completion time frame falls between 24-72 hours once the prior authorization is submitted to your insurance and incurs a $25 fee. Your pharmacy will tell you if a particular prescription drug requires a prior authorization. It is their responsibility to contact our office to request that a prior authorization be initiated.

Please be advised that if your insurance company requires additional information, your prior authorization may take several weeks to complete. A prior authorization DOES NOT guarantee coverage. We do not have control over your prescription drug coverage program. If your prior authorization is denied, please contact your insurance company for questions or complaints.
